Evaluate xrays using Elbow Outcome Measures. Severe elbow pain and limited arm movement left. Consider bone stimulator if union not evident on xrays. Complex open reduction and internal fixation (ORIF) surgery enables Michael to enjoy golf and tennis again. Begin gentle sport specific exercises depending on fracture union. 3 Months: Evaluate xrays for fracture union.Place in removable posterior splint and begin elbow ROM exercises. 6 Weeks: Wound check, evaluate xrays for callus formation.Place in long-arm cast or removable posterior splint depending on fracture type and strength of fixation. 10-14 Days: Wound check, remove staples.
LEFT OLECRANON FRACTURE SERIES
Post-op: Place in posterior splint in 50-70º of flexion. PMID: 27211227 DOI: 10.1016/j.injury.2016.04.015 Abstract Introduction: The aim of this study was to report the physical and functional outcomes after open reduction internal fixation of the olecranon in a large series of patients with region specific plating across multiple centres.Draining sinues / wound healing complications.Incise triceps and impact bent ends into the proximal ulna with bone tamp.Trim twisted ends and bend into the adjacent soft tissue. Tighed the wires both medially and laterally.Place one 18-gauge wire or two 22-gauge wires (using two tranvserse distal drill holes) in figure-8 fashion through transverse hole and under the triceps insertion and k-wires proximally.Place 2.5mm drill hole transversely across triangular ulna diaphysis distal to the fracture and anterior enough to be bicortical.Back them up 5mm to allow later impaction. Place two parallel 0.062 or 0.045 k-wires obliquely from proximal ulna, across fracture site and into the anterior ulnar cortex.Consider drill hole in dorsal ulna to provide anchor point for clamp.
LEFT OLECRANON FRACTURE SKIN
The olecranon is located under the skin of the elbow, without much protection from muscles or other soft tissues. The post-fall condition shows an acute, nondisplaced fracture of the olecranon process. This pointy segment of bone is part of the ulna, one of the three bones that come together to form the elbow joint. Olecranon fracture is a common injury, representing 8-10 of all elbow fractures 1, it's an intra-articular fracture and can cause disruption of the elbow extensor mechanism, therefore most of these fractures are treated surgically. This exhibit depicts two lateral conditions of a left elbow fracture.
